Registering at Parkfield is a simple process, please complete the new patient registration form. We do not ask for ID, we only ask that you live within our Practice Boundary.

Once you have completed the form your registration will take up to 5 working days to process.

Please ensure that you have enough medication (at last one month) to last until your registration has been processed.

Practice Area

The practice area is clearly defined. The practice area includes: Potters Bar, Brookmans Park, Welham Green, Wildhill, Tylers Causeway, Newgate Street, Northaw, Botany Bay, Hadley Wood, Hadley Highstone, South Mimms and Ridge.

Change of address, email address and phone numbers

If you move, change your email address or telephone number(s) please let us know so that we can keep our details up to date. If you need to update your details, you can do so online by completing the Change Personal Details triage.

Change of name

If you change your name, you will need to provide a copy of a marriage certificate, deed poll etc.
