Patient Participation Group Registration
The Friends of Parkfield Medical Centre Patient Participation Group (PPG) was formed in October 1998.
The overall aims of the PPG are to develop a positive and constructive relationship between patients and the practice, and to inform patients about health care by providing talks and discussions, by publishing newsletters and providing information about NHS initiatives.
An annual donation of £2.00 per adult or £3.00 family membership is suggested (or more, by choice). This money will go towards the provision of information, equipment, additional services for patients, and regular newsletters for distribution to members. We are a registered charity (No 1075710).
Persons wishing to join the PPG may do so providing they are 18 or over and patients of Parkfield Medical Centre.
If you are a UK taxpayer we can use Gift Aid to recover tax on your donation and boost this by 25p for every pound you donate. All you need to do is sign and date the Gift Aid declaration overleaf. Many have chosen to contribute more than the suggested donation and we are very grateful for this as it allows us to give much more significant support to the Practice through the provision of resources, making a visit a better experience for Parkfield’s patients.
